
Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF) in Duluth, MN
Job Description
We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.
• Perform patient assessments, diagnose, and manage acute and chronic conditions.
• Order, interpret labs, develop treatment plans, and prescribe medications within guidelines.
• Educate patients, families, and staff to promote health and prevention.
• Collaborate with care teams to ensure seamless, quality care delivery.
• Maintain accurate documentation and support quality improvement initiatives.
Qualified candidates will hold a Minnesota Nurse Practitioner license, be board-certified, and preferably have over a year of experience in a skilled nursing or similar setting. The role offers flexible scheduling, autonomy, a competitive salary ($100,000–$140,000), and benefits including health insurance, CME allowance, paid time off, and professional development opportunities.
