
Nurse Practitioner - Nashua, New Hampshire in Nashua, NH
Job Description
Provide expert healthcare in nursing home and assisted living settings by delivering comprehensive care designed to improve patient outcomes and reduce unnecessary hospitalizations. This role focuses on treating insurance members within skilled nursing and assisted living facilities, promoting effective transitions to home when appropriate. Regular health assessments, performed at least monthly, ensure timely interventions and resource allocation to support patients well-being and avoid preventable emergency visits.
Schedule:
• Monday to Friday, 8 AM to 5 PM
• No weekends or holidays
Key Responsibilities:
• Develop and maintain problem lists and medical treatment plans for each resident
• Deliver primary, preventative, urgent, and acute care on-site with hands-on bedside management
• Identify and stratify high-risk patients for more frequent clinical visits
• Create contingency plans for patients with high-risk conditions
• Collaborate effectively with primary care physicians and facility staff, including Directors of Nursing
• Conduct routine patient care visits and assessments across multiple nursing or assisted living facilities
• Perform single visits for patients transitioning from hospital discharge to ensure a safe and healthy recovery
Qualifications:
• Master of Science in Nursing (MSN) degree
• Current New Hampshire Nurse Practitioner License
• Basic Life Support (BLS) certification
• DEA registration
• Experience in long-term care settings is preferred
