The Nurse Practitioner plays a key role in supporting patients and their families during advanced illness by providing skilled medical care and calm, compassionate guidance. They help families understand care options and what to expect, supporting decisions that reflect the patient’s values and goals. Through oversight of medications, treatments, and recertifications, the Nurse Practitioner helps ensure continuity of care, allowing families to focus on comfort, dignity, and meaningful time together. Qualifications Nurse Practitioner Requirements & Key Responsibilities: • Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice. • Degree from an accredited NP program required. • Minimum two years of RN experience; preferred hospice or palliative care experience. • Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans. • Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations. • Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care. • Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs. • Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives. • Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records. • Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
