This opportunity involves providing specialized care to residents in nursing home and assisted living settings. The focus is on delivering comprehensive, on-site primary, preventive, urgent, and acute care that promotes quality outcomes and reduces unnecessary hospital visits. Regular health assessments are conducted, typically at least monthly, to monitor and address patient needs effectively, facilitating smoother transitions back to home environments when appropriate. Schedule: • Monday through Friday • 8 AM to 5 PM • No weekends or holidays Key Responsibilities: • Develop individualized problem lists and medical treatment plans for each resident • Provide hands-on bedside care and comprehensive routine patient care at assigned facilities • Stratify high-risk patients to prioritize frequent clinical evaluations • Create contingency plans for patients with high-risk conditions • Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing • Conduct transitional visits for patients recently discharged from the hospital to ensure a safe and healthy recovery within the facility • Manage a panel of members across multiple nursing or assisted living facilities, typically scheduled at one facility per day, with flexibility for additional visits as needed Qualifications: • Master of Science in Nursing (MSN) degree • Current Maryland Nurse Practitioner license • Basic Life Support (BLS) certification • DEA registration • Experience in long-term care preferred About Delmar, MD: Located along the border of Maryland and Delaware, Delmar offers a peaceful community atmosphere with convenient access to larger metropolitan areas.
