Job Description:
Hill Health, a Hill Physicians Practice, is looking for a Nurse Practitioner to join our Oakland based primary care team. This patient centric practice was established in late 2015 and is in a freshly designed space across from Summit Medical Center.
The Nurse Practitioner is responsible for working collaboratively to support physicians, nurse practitioners, other healthcare professionals and the Practice Administrator to provide primary health care to acute and chronic patients. They should exhibit behavior and communication skills that demonstrate commitment to superior patient experience including quality, care and concern with each and every internal and external customer.
FUNCTIONS:
- Actively sees patients that require same day appointments for acute illnesses. May continue to follow and see patients with chronic conditions.
- Working in collaboration with other nurse practitioners and the physicians, has responsibility for Annual Wellness Visits and Advanced Care Planning.
- Provides support to physicians including but not limited to:
- Triaging patient issues (phone calls)
- Prescription refills
- Monitoring tasks for physicians out on PTO
- Providing guidance to Medical Assistants
REQUIREMENTS:
- Master’s degree in nursing
- Active, unrestricted California Nurse Practitioner license
- Current DEA license required
EXPERIENCE:
- Minimum three years clinical nursing experience in a primary care setting
KNOWLEDGE, SKILLS, ABILITIES:
- Strong patient care orientation
- Ability to handle stressful situations in a positive and professional manner
- Flexible and independent
- Familiar with EHR, preferably Epic
- Strong leadership, organizational and communication skills
- Chinese speaker highly desired
Additional Information:
Compensation:
$150,000.00 to $170,000.00 annual
