Position DescriptionHarlem United is the parent company for Upper Room AIDS Ministry (URAM). URAM provides the healthcare services from the Federally Qualified Health Center located in the heart of Harlem. We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible to perform their job practicing good risk management, quality assurance, and delivering excellent internal and external customer service. The Nurse Practitioner assists with leading the care team and provides guidance to all team members. The Nurse Practitioner is expected to help lead the care team with a commitment to ensuring each team works to the top of his or her license consistently adheres with all protocols. It is expected that the Nurse Practitioner will delegate visits that do not require a physician or Nurse Practitioner to the RN allowing the Nurse Practitioner to dedicate more time to those patients that need to be seen with the scope of practice for the Nurse Practitioner. Nurse Practitioner’s are also expected to ensure that each patient has an individualized care plan with oversight by the PCP and that can be used by each care team member to help facilitate and guide care. Nurse Practitioner are also responsible to provide excellent customer service at all times.Under the supervision of the Medical Director and in accordance with established policies and procedures, the Nurse Practitioner delivers high quality comprehensive primary care, with expertise in HIV, immigrant and homeless care. The AIDS institute Guidelines and the U.S. Preventive Services Task Force (USPSTF) are utilized as resources and guides for accepted standards of care.Essential Job FunctionsThe following duties are mandatory requirements of the job:Provide excellent customer service at all times, apologizing for any throughput delays, engaging patients with a warm and friendly greeting.Provide direct patient care, including prevention, screening, diagnosis, and treatment of illness, within their licenses scope of practice and specialty trainingProvides Primary Care, routine and acute care, for scheduled, same day and walk in appointments for face to face encounters as well as telehealth encounters. Conduct visits per protocol for all patients with chronic conditions under the direction of the physician and addresses urgent and complex issues. Including Initiation of treatment and management of chronic illnesses including, but not limited to, HIV infection, Hepatitis C, substance abuse, asthma, bronchitis, bronchitis, diabetes, lipid disorders, and hypertension.Identify and delegate visits that can be handled by RN under the supervision of the physician and having notified the patient in advanceMaintains a professional demeanor at all times, including demonstration of sound moral and ethical character, compassion, adherence to all agency standards for dress and decorum; reports for duty and signs in and out per agency protocol.Demonstrates proficiency of electronic medical record; including documentation, e-prescribing and coding.Complies with productivity expectations under the direction of the Medical Director.Delivers cost effective care, according to evidence based guidelines Generates appropriate referrals to ancillary and specialty services including medical, mental health and social based services.Maintains documentation and e-prescribing practices according to standards of the agency and regulatory agencies including but not limited to history and physical, social history and literacy level as well as care management.Completes and locks progress notes within 48 hours of client encounter.Ensure timely prescribing, orders, addressing labs, interpreting of test results, addressing telephone encounters, ordering and reviewing of specialty referrals and consultation reports.Ensure individualized care plan contains care recommendations, recommend frequency of visits, schedule for tests/screenings and red flags/plan of actionPerforms routine diagnostic and therapeutic procedures according to accepted practice standards.Assist with assigned PCP’s panel in managing lab results, reviewing consult reports, and reviewing consultation reports as determined by the PCP.May administer medications according to the same procedure and protocols applied to the nursing staffAssists in communicating the management of transitions of members with complex care needs. Discusses and refers complicated patients outside of clinical expertise to appropriate physician.Provide health education and counseling during visits.Provides on-call telephone coverage on a rotating schedule.Assists with direction of care team with commitment to all team members practicing to the top of their license Follow all protocols for daily huddle Participates in CQI activities.Attend patient care related meetings as necessary, including but not limited to morning report, case conferencesManage coordination of care for individual clients by working with the available resources both internal and external to agency Other duties as assigned by the Medical Director.Other ResponsibilitiesThe following duties are to be performed as assigned by the supervisor:Assists with oversight of care team and be a leaderParticipate in Meetings including Quality of Care, Clinical Training, Business, Peer Review and Journal Club meetingsAttends conferences and community based activities, including advocacy efforts to eliminate homelessness, HIV and disparities in care as designated by the Medical Director.Education and Certification Graduate of an Accredited Nurse Practitioner Program Licensed and Registered in New York StateCertified by either the American Academy Nurse Practitioners or American Nurses Credentialing Center (ANCC)BLS CertifiedAchieves and maintains HIV specialist statusPossess or obtain within first 90 days of employment a DATA Waiver for Buprenorphine Prescribing authorityAccumulate a minimum of 15 HIV specific CME credits throughout the year and a minimum of 25 Credits per year to maintain certification requirementsSpecial Skills and KnowledgeIn addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:Bilingual preferred (English/Spanish or English/French).Experience in treating hard to reach populations, including the homeless, immigrants, HIV infected, substance users, and mentally ill.Desire and ability to work as part of an interdisciplinary team dedicated to the delivery of high quality care.Leadership abilities with trouble shooting and problem solving skills.Proficient in use of electronic medical record systems and other technology required for use in the delivery of care.Track record of positive interpersonal skills with staff, clients, and others.Willingness to work off hours on special projects as reasonably dictated by Medical Director.The spirit and desire to-practice in a community based interdisciplinary, mission driven system of care.Special RequirementsThe employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination. Requires sitting, standing and walking for extensive periods. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
