On Call-After Hours- 7on/7off Hospice Registered Nurse- RN- Salt Lake City in Millcreek, UT

Rocky Mountain Care is seeking AMAZING RN's to provide hospice care and case management for our patients in the Salt Lake City area!

This is an On Call-After Hours- 7on/7off schedule

Responsibilities include:
• Admit patients to home care services
• Establish and carry out a plan of care
• Communicate effectively to interdisciplinary team members
• Case management
• Observes/ensures compliance with Medicare and State guidelines
• Discharge planning for Home Care patients
• Attends all IDT and mandatory meetings

Position Requirements:
• Must have a Valid UT RN License
• Reliable Transportation is Required
• Must Possess Clinical, Organizational, and Communication Skills
• Must have at least 3 years of experience in hospice, or several years in nursing.

Pay: $36.00 - $39.00 per hour
