Position: On Call Hospice Registered Nurse
Position Type: Full-Time
Coverage Area: Waterbury/Southbury areas
Salary: $90,000 - $105,000/year
Schedule: Every Saturday and Sunday On Call plus 1 day during the week (Monday, Tuesday, or Thursday)
Work Shifts: 3 shifts of 12 hours from 7am - 7pm or 8am - 8pm

Qualifications:
• Current licensure as a Registered Nurse is essential, with an associate degree preferred. CPR certification is recommended and prior experience in hospice, home health, medical-surgical, or nursing in geriatrics or pediatrics is desirable.
• A willingness to join an on-call rotation is required, along with reliable transportation and valid auto insurance.
• Strong communication and organizational skills are vital, with proficiency in Microsoft Office Suite and EMR systems.
• Physical capabilities to engage in regular activities which may include lifting patients over 200 lbs with assistance and handling supplies and equipment weighing up to 20 lbs are necessary.
Your Role & Responsibilities:
• Conduct comprehensive nursing assessments and collaborate with patients, families, and the Interdisciplinary Group (IDG) to create and manage individualized care plans.
• Document accurately and timely, ensuring care plans reflect current patient conditions and facilitate effective communication with the care team.
• Provide education and emotional support tailored to individual patient needs and actively participate in on-call duties and care transitions.
• If serving as a Care Manager, oversee scheduling, medication management, and collaboration with the IDG while supervising Hospice Aides.
• If in an On-Call role, handle triage calls, conduct visits and assessments, and coordinate services with the Administrator On Call.
