Job Description

As an After Hours Hospice RN, you will play a vital role in responding to patient needs during evenings, nights, weekends, and holidays, offering peace of mind and compassionate care when it matters most.
Your Responsibilities as an On-Call Hospice RN:
• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
• Provide direct patient care and support after normal business hours.
• Act as the primary link between patients, families, physicians, and the hospice team during your shift.
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs.
• Assess patient and family requirements to inform clinical decisions and implement the care plan.
• Collaborate with an interdisciplinary team, including LPNs/LVNs, CNAs, and support staff.
• Document clinical visits and communications accurately.
• Participate in quality improvement programs.
• Work independently while ensuring seamless care coordination with the daytime team.
About You:
• Current RN license in the state of your employment (or eligibility to obtain).
• 1+ years of nursing experience.
• A heartfelt commitment to hospice care, exhibiting compassion, calmness, and reliability in urgent situations.
• Demonstrated clinical judgment and ability to provide support in emotional settings.
• Comfort in delivering care in patients' homes, facilities, or wherever they reside.
• Valid driver's license, automobile insurance, and reliable transportation.
• Current CPR certification.
Preferred Qualifications (Not Required):
• 1+ years of hospice, home health, oncology, or palliative care experience.
• Knowledge of symptom management, triage, and after-hours home-based care.
• Prior experience in on-call or overnight nursing roles.
• Strong skills in documentation and electronic medical records (EMR).

Salary Range: $71,450 - $89,335
