
On-Call Hospice Registered Nurse (RN) in Wilmington, NC
Job Description
Overview: Lead with Heart. Make a Difference. Provide Compassionate Care After Hours. We offer a full-time Weekend Baylor schedule complete with benefits and paid time off.
As an After Hours Hospice RN, you will respond to patient needs during evenings, nights, weekends, and holidays—bringing peace of mind and compassionate care when it's needed most.
What You Will Do:
• Report directly to the Administrator or Patient Care Manager
• Deliver direct patient care and support after normal business hours
• Act as the main liaison among patients, families, physicians, and the hospice team during your shift
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs
• Evaluate patient and family needs to inform clinical decisions and implement the plan of care
• Collaborate with an interdisciplinary team including LPNs/LVNs, CNAs, and other staff
• Document clinical visits and communications accurately
• Participate in quality assessment and performance improvement programs
• Work independently while ensuring continuity of care and cooperation with the daytime care team
About You:
Experience - What You'll Bring:
• Current RN license in the relevant state (or eligibility to obtain)
• At least 1 year of nursing experience
• A compassionate disposition suitable for hospice care: calm and dependable in urgent situations
• Strong clinical judgment with the ability to support in emotionally charged environments
• Comfortable providing care in patients' homes or other facilities
• Valid driver's license, auto insurance, and reliable transportation
• Current CPR certification
Preferred Experience (not mandatory):
• 1+ years in hospice, home health, oncology, or palliative care
• Familiarity with symptom management and after-hours home-based care
• Prior experience in on-call or overnight nursing roles
• Strong documentation and electronic medical records skills
