Schedule: Two-week rotation from 5 PM to 3 AM on weekdays and 8 AM to 8 PM on weekends, including:
• Monday, Tuesday, Saturday, Sunday
• Wednesday, Thursday, Friday

Your Responsibilities:
• Manage after-hours patient and family phone calls to determine the need for services such as assessments, symptom management, and care plan updates. Delegate nursing visits accordingly.
• Collect information on physical, psychological, social, and spiritual factors affecting patients and their families, facilitating interventions from the interdisciplinary team when needed.
• Coordinate with attending physicians and other staff members to ensure seamless and optimal patient care.
• Perform clinical pronouncement of death and provide comforting support to the family.
• Maintain accurate clinical records through timely care plans and progress notes.
Qualifications:
• Graduate from an accredited nursing program
• Current registered nursing license in the state
• Minimum two years of professional nursing experience
• Excellent clinical judgment and nursing skills
• Ability to handle emotional stress and respect individual lifestyles
• Current CPR certification required
• Valid driver's license and proof of insurance
