Join our innovative hospice and palliative care team, where we are dedicated to making a meaningful difference in the lives of patients facing life-limiting conditions. We offer a variety of community and facility-based services designed to support both patients and their families in navigating the challenges of serious illness and end-of-life care. As an On-Call Registered Nurse, you will play a crucial role in providing compassionate care during after-hours shifts. Your responsibilities will include: • Conducting scheduled and unscheduled patient visits for urgent physical and psycho-social issues • Coordinating with the Call Center Triage and On-Call Manager to respond promptly to visit requests • Communicating effectively with the on-call team and Hospice physician to address urgent patient needs • Implementing and educating patients and their primary caregivers about changes to the plan of care, including medication management and symptom control • Offering emotional support and guidance to families during patient deaths, as directed • Reporting patient-related visits, changes to care plans, and follow-up needs to the triage and primary Hospice team To qualify, you should have: • A degree from an accredited School of Nursing • At least two years of experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing preferred • Current New York State RN licensure and a valid NYS driver's license preferred • A commitment to sit for the national CHPN certification exam within one year of hire • Proficiency in Word, Excel, and Windows applications If you are passionate about making a difference in the lives of others and meet the qualifications listed above, we encourage you to apply!
