
On-Call Registered Nurse in Oceanside, NY
Job Description

As an On-Call Registered Nurse, you will:
• Provide prompt patient visits for urgent physical and psycho-social concerns during after-hours shifts.
• Coordinate effectively with the Call Center Triage and On Call Manager, responding to visit requests via cell phone.
• Communicate with the on-call team, including Hospice physicians, to address urgent patient needs and create a safe plan of care.
• Implement care plans, educating patients and primary caregivers about any changes, including medication management, patient safety, and symptom management.
• Offer emotional support and guidance to families during patient deaths as needed.
• Document patient visits and communicate with the primary Hospice team regarding care changes and follow-up needs.
Requirements:
• Graduation from an accredited School of Nursing.
• A minimum of two years of experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology Nursing is preferred.
• Active New York State RN licensure and a valid New York State driver’s license is preferred.
• Commit to obtaining national board certification in Hospice and Palliative Nursing (CHPN) within one year of hire.
• Familiarity with Word, Excel, and Windows applications is necessary.
