Job Description

As an On-Call Registered Nurse, you will play a crucial role in providing patient care during after-hours shifts. Your responsibilities will include:
• Responding to scheduled or unscheduled patient visits for urgent physical and psycho-social issues.
• Collaborating with the Call Center Triage and On-Call Manager to address patient needs effectively.
• Communicating with the on-call team and the Hospice physician to ensure timely care and a safe plan of action.
• Implementing care plans while educating patients and primary caregivers on medication management, patient safety, and symptom control.
• Providing emotional support and guidance to families during patient deaths as directed.
• Reporting to the triage and primary hospice team regarding visit outcomes and follow-up needs.
Qualifications:
• Graduate from an accredited School of Nursing.
• A minimum of two years’ experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing is preferred.
• Current New York State RN licensure and a valid NYS driver’s license preferred.
• Must plan to sit for the national CHPN certification within one year of hire.
• Proficiency in Word, Excel, and Windows applications.
