On-Call Team Nurse Practitioner or Physician Assistant Summary Dedicated position. Nights, weekends, and holiday rotations | .6 FTEThis role is focused on providing both acute and chronic care management for adult and geriatric patients. The On-Call Nurse Practitioner or Physician Assistant collaborates with supervising physicians to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life. This role requires the ability to work collaboratively with a multidisciplinary healthcare team and stay updated on best practices in senior care.Key ResponsibilitiesProvide general medical care to various and/or specific patient levels- adults/geriatrics. Address both acute and chronic primary care complaints and ascertain medical urgency. Interpret labs/diagnostics- treat as needed within scope of practice. Document in Electronic Health Record (EHR) Discuss unclear or unusual symptoms or results with supervising physician. Prescribe medications as appropriate.
