Oncology Nurse - Transitions of Care in Montgomery, AL

A company is looking for an Oncology Transitions of Care Nurse. Key Responsibilities Conduct clinical assessments and monitor changes in health for members diagnosed with cancer Coordinate care transitions and educate members and caregivers about diagnosis and treatment Assist with administrative projects and communicate feedback to improve service offerings Required Qualifications Bachelor of Science in Nursing (BSN) and a compact unrestricted Registered Nurse (RN) license Minimum of 5 years of nursing experience, including 3 years in oncology nursing or case management Certified Case Manager (CCM) certification required Ability to work in a fast-paced environment and manage multiple tasks effectively Willingness to learn and use new technologies, including electronic health records and communication tools
