Job Summary

As a Registered Nurse, you will work under the guidance of our nursing leadership and collaborate with healthcare providers to deliver evidence-based care in accordance with the State Nurse Practice Act and Hartford Healthcare’s policies. Your role will encompass addressing the physical, emotional, spiritual, and educational needs of our patients and their families.
• Provide nursing services within a supportive, non-acute setting in collaboration with interdisciplinary teams.
• Coordinate and deliver holistic, patient-centered care tailored to individual needs.
• Utilize evidence-based practices to ensure safe, high-quality care and exceptional customer service.
• Incorporate the four pillars of our Professional Practice Model: Excel, Empower, Advocate, and Engage.
• Support research initiatives by inquiring about and applying evidence-based practices.
• Evaluate and adapt care plans through active collaboration with the interdisciplinary team to meet diverse patient needs.
• Identify and document patient issues and expected outcomes based on established care plans.
• Maintain confidentiality of all patient information.
• Support and facilitate two clinical therapeutic groups daily.
• Lead daily activity sessions for patients.

Qualifications
• Bachelor's degree in Nursing from an accredited program.
• Completion of a nurse residency program, if applicable.
• One year of clinical experience as a licensed RN (minimum 1000 hours); five years preferred.
• Current Registered Nurse license in the State of CT.
• Certified BLS Provider credential from the American Heart Association (AHA).
• Additional specialty certifications are preferred and may be required.
• Effective communication skills in English.
