Responsibilities Of The Nurse Care Manager • Developing and supporting integrated care plans with SPMI population in community setting • Coordinate somatic & behavioral health care through communication with internal & external providers • Utilize nursing experience and clinical knowledge to educate clients and staff on techniques to better manage their somatic conditions Qualifications Of The Licensed Practical Nurse • Bachelor’s Degree from an accredited School of Nursing • Active Maryland RN licensure required • 2 years of relevant work experience plus one year of supervisory experience preferred Benefits Offered To The Licensed Practical Nurse • Pay Range - annually based on experience • Comprehensive benefits including medical, dental, & vision • Three weeks of PTO in the first year Cornerstone Montgomery is an equal opportunity employer.
