Key Responsibilities:
• Deliver personalized patient care and manage cases for hospice patients in outpatient settings.
• Ensure that end-of-life care is delivered to meet quality and utilization standards, focusing on achieving positive clinical outcomes for patients and their families.
• Develop, coordinate, and manage family-centered care plans with a multidisciplinary team for patients in home or community settings.
• Maintain timely documentation that meets regulatory standards.
• Conduct comprehensive assessments of patients' physical, mental, social, and spiritual needs.
• Provide skilled nursing care, organizing, directing, and documenting care according to the care plan.
• Evaluate the effectiveness of care interventions and adjust as needed based on patient responses.
• Maintain efficient communication with the multidisciplinary team to enhance care delivery and ensure that care plans are followed accurately.
• Act as a patient advocate and mentor newly hired nurses or students.
• Participate in quality improvement initiatives and maintain productivity standards.
Qualifications:
• Associate Degree in Nursing
• Active State of New Mexico or Compact State Nursing License
• BLS certification REQUIRED at the time of hire
• Minimum one year of direct patient care experience; home health care preferred
• Acute care experience preferred within the last three years
• Hospice care experience is a bonus
• Current driver's license and auto insurance
