
Overnight Triage Nurse - Registered Nurse (Full Time) in Bethlehem (CDP), NH

Looking for a unique remote position that allows for great work life balance. NCHHHA agency is seeking a Registered Nurse for our overnight remote triage position.
Position Summary
The Overnight Home Health and Hospice Triage Nurse provides after-hours telephone triage and support for patients, families, caregivers, providers and staff. This position ensures 24/7 clinical coverage by assessing patient concerns, coordinating care, and dispatching field staff when needed. The triage nurse acts as a critical communication link between patients, caregivers, on-call providers, and the interdisciplinary team.
Key Responsibilities
• Telephone Triage
• Answer incoming calls promptly and professionally during overnight hours.
• Assess patient symptoms, caregiver concerns, and urgent clinical needs.
• Provide safe, evidence-based instructions for care at home.
• Clinical Decision-Making
• Determine the appropriate level of intervention needed.
• Coordinate and dispatch on-call field nurses or staff when necessary.
• Escalate critical cases to the on-call physician or advanced practice provider.
• Care Coordination
• Document all calls and interventions accurately in the EMR.
• Communicate changes in patient condition to the day-shift team for follow-up.
• Patient & Family Support
• Provide emotional support, guidance, and reassurance to patients and families in crisis.
• Reinforce education regarding disease process, medications, and care expectations.
• Ensure safety by reminding caregivers of emergency protocols.
Qualifications
• Current RN license in the state of New Hampshire
• Minimum 2 years of clinical nursing experience, preferably in home health, hospice, or acute care- willing to train the right candidate.
• Strong clinical assessment and critical thinking skills.
• Autonomous
• Excellent telephone communication skills and ability to remain calm in stressful situations.
• Proficiency with EMR systems and remote working.
• Must live in NH, VT or North Carolina
Schedule
• Overnight shift (4:30p-8:00a) Monday- Friday
• Rotating weekends and holidays as required to cover colleague time off
• This is a salaried position
