
Palliative Nurse RN in Des Moines, IA
Job Description
In this role you will...
• Deliver compassionate, comprehensive care by educating patients, managing medications, assessing health, and providing emotional support to patients and families.
• Coordinate interdisciplinary care plans by collaborating with nurse practitioners, social workers, and other professionals, while triaging patient needs and guiding advance care planning.
• Ensure seamless transitions and compliance by supporting intake and referrals, maintaining accurate documentation, and facilitating hospice care when appropriate.
• Travel in all weather conditions to patients homes in the assigned territory.
Qualifications:
• Valid, unrestricted Iowa driver’s license.
• Current vehicle insurance.
• Current license as a registered nurse in the state of Iowa.
• Two years of full-time nursing experience required, with one year of hospice experience preferred.
