Part-Time CAP Registered Nurse (RN)
Job Type: Part-time (PRN)
Position Summary
As a CAP Registered Nurse (RN), you will play a crucial role in providing in-home nursing assessments and managing care plans for beneficiaries in the Community Alternatives Program (CAP/DA and CAP/C). This position supports adults and children with disabilities in achieving independence and avoiding unnecessary institutionalization. You'll conduct comprehensive assessments, maintain accurate electronic medical records, and work in close collaboration with CAP Case Managers to ensure our patients receive safe, coordinated care.
Compensation Structure (Unit-Based)
This PRN role features a unit pay model, ensuring predictable earnings based on the type of nursing assessment and travel requirements.
• Base Unit Rate: $43.75 per unit
Paid Unit Types
• Continued Need Review (CNR) Assessment
Weight: 2.5 units
Compensation: $109.38 (2.5 x $43.75)
• Change of Status (COS) Assessment
Weight: 3 units
Compensation: $131.25 (3 x $43.75)
• Travel Unit
A travel unit is awarded for travel time to a beneficiary's residence taking 1 hour or more, or for travel time between two beneficiary locations taking 1 hour or more.
Compensation: $43.75 per travel unit
(Note: A travel unit applies only for the trip to the visit, not the return trip.)
Mileage Reimbursement
• $0.50 per mile for all eligible travel related to beneficiary assessments.
This compensation structure provides clear and consistent pay for both clinical work and travel-related tasks.
Key Responsibilities
• Conduct initial, annual, CNR, and COS nursing assessments in beneficiaries’ homes.
• Evaluate the medical, psychological, social, and environmental needs of beneficiaries.
• Create and update person-centered care plans and recommend suitable interventions.
• Document updates and maintain care plans in the electronic medical record (E-CAP).
• Collect medical records from providers to ensure continuity of care.
• Review records after critical incidents for quality assurance.
• Collaborate with CAP Case Managers, family members, and healthcare providers.
• Ensure all nursing documentation complies with state regulations and CAP audit standards.
Requirements
• Current North Carolina RN license (mandatory).
• Experience in assessments, care planning, or community-based nursing is preferred.
• Strong organizational, communication, and documentation skills.
• Ability to work independently in a field-based setting.
• Reliable transportation is required as 100% travel within the assigned region is necessary.
Physical Requirements
• Frequent driving, entering/exiting homes, navigating stairs, and carrying light supplies.
• Ability to engage in extended periods of computer-based documentation.
