Position Summary
The CAP Registered Nurse (RN) plays a vital role in providing in-home nursing assessments and overseeing care plans for beneficiaries in the Community Alternatives Program (CAP/DA and CAP/C). This position is dedicated to supporting disabled adults and children in maintaining their independence while avoiding unnecessary institutional care. The RN will conduct comprehensive assessments, update electronic medical records, gather essential documentation, and work closely with CAP Case Managers to ensure that beneficiaries receive safe and coordinated care.
Compensation Structure (Unit-Based)
This PRN role utilizes a unit pay model that provides predictable pay for each type of nursing assessment and required travel.
• Base Unit Rate: $43.75 per unit
Paid Unit Types
• Continued Need Review (CNR) Assessment
Weight: 2.5 units
Compensation: $109.38 (2.5 x $43.75)
• Change of Status (COS) Assessment
Weight: 3 units
Compensation: $131.25 (3 x $43.75)
• Travel Unit
Awarded when travel time to a beneficiary's residence is 1 hour or more, or when travel time between two beneficiary locations is 1 hour or more.
Compensation: $43.75 per travel unit
(Note: A travel unit applies only for the trip to the visit, not the return trip.)
Mileage Reimbursement
• $0.50 per mile for all eligible travel associated with beneficiary assessments.
This structured compensation model guarantees clear and consistent pay for both clinical and travel-related tasks.
Key Responsibilities
• Perform initial, annual, CNR, and COS nursing assessments in beneficiaries' homes.
• Assess medical, psychological, social, and environmental needs.
• Create person-centered care plans and suggest appropriate interventions.
• Maintain accurate documentation and care plans in the electronic medical record (E-CAP).
• Collect medical records from providers to ensure continuity of care.
• Review records following critical incidents.
• Collaborate effectively with CAP Case Managers, families, and healthcare providers.
• Ensure nursing documentation adheres to state requirements and CAP audit standards.
Requirements
• Current North Carolina RN license (required).
• Experience in assessments, care planning, or community-based nursing is preferred.
• Exceptional organizational, communication, and documentation skills.
• Ability to work independently in a field-based role.
• Reliable transportation as 100% travel within the assigned region is necessary.
Physical Requirements
• Frequent driving, entering/exiting homes, navigating stairs, and carrying light supplies.
• Extended periods of computer-based documentation.
