• 8am to 5pm

As an RN in this role,you'llwork both independently and as part of an interdisciplinary team to conduct comprehensive assessments, develop and implement individualized care plans, and deliver outcome-driven interventions under the physician-ordered Plan of Care.

The registered nurse is accountable for the delivery and supervision of safe, quality care that is individualized for their assigned patients and follows the nursing processin accordance withthe Nurse Practice Act (assessment, planning implementation, evaluation).
Position Specific Responsibilities
• Possesses knowledge of developmental needs and in the assessment and treatment as it relates to the age of the patient population served.
• Conducts and documents a comprehensive patient assessment upon admission.
• Continually reassesses and appliesappropriate advocacyand interventions asindicated.
• Competentin the interpretation of a patient's self-assessment and behavior. This information must be interpreted with an understanding of the cognitive,physicalemotional,psychosocialand chronological maturation process.
• Collaborates with the patient,familyand members of the healthcare team; develops, documents, implements andmonitorsan individualized plan of care which reflects a standard of care based on best practice and evidence-based medicine.
• Adaptive tochangeandupdatedpatient care practices within an evolving healthcare environment.
• Legibly and electronically documents patient care activities and information inan accurate, concise, andtimelymanner.
• Supports andassistswith orientation ofnew staffand students.
• Acquiresnecessary training and skills to competently perform the preceptor and charge nurse function when selected and functioning in those roles.
• Demonstrates advancement of professional nursing practice through continuing education, required training, certification, and participation in departmentactivities.
• Takes an active role in building andmaintainingan environment that fosters open communication, patient and family-centered care and healthy collegial relationships.
• Demonstrates adaptive/timelycommunication based on individual patient/family needs.
• Attends andparticipatesin a minimum of 50% of scheduled staff meetings or has read the minutes and signed off onthem andis responsible formeeting content regardless of attendance.
Education and/or Experience
• Required-Associate or Bachelor'sdegree froman accreditedSchool ofNursing.
• CA:Required -1 yearProfessional nursing experience.
• CA: Preferred - Home Health field experience.
• CA: Preferred - IV and Central Line experience
• CA: Preferred - Experience with wound care including negative pressure devices.
Certifications, Licenses, and Registrations
• Required-Current, unencumbered(Stateof Employment)License as a Registered Nurse.
• Required-National Provider BLS - American Heart Association (Vendor Managed)
Physical Demands and Work Environment:
The demands of this rolenecessitatea team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $50.18 - $77.91
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
