The Home Infusion Registered Nurse plays a vital role in delivering clinical care and education to patients receiving various parenteral medications from Optum Infusion Pharmacy. This position primarily involves intravenous administration, although other routes such as subcutaneous and intramuscular injections may also be required. You will execute services primarily in patients' homes and, where available, in ambulatory infusion suites, adhering to established protocols and guidelines.
• Build and maintain positive relationships with patients and their families to facilitate infusion therapy and provide education on self-care and self-administration.
• Evaluate the suitability of patients for infusion therapy, assessing their home environment, caregiver support, and readiness for self-monitoring.
• Gather and maintain comprehensive patient medical histories and clinical documentation, ensuring all records are timely and thorough.
• Collaborate in case conferences with interdisciplinary teams to devise comprehensive care plans.
• Provide skilled nursing care as specified in treatment plans, ensuring adherence to physician orders.
• Perform vascular access procedures and administer medications as prescribed while monitoring patient reactions and side effects closely.
• Communicate patient observations and responses to therapy accurately with the multidisciplinary care team.
• Nurture relationships with home health agencies for effective care delivery and provide support and training as required.
• Stay informed on infusion therapies and adhere to protocols, ensuring high-quality patient care.
• Maintain an active nursing license and ensure compliance with all safety, privacy, and regulatory standards.
• Participate in training and orientation activities to enhance team knowledge and skillsets.
• Utilize your own or rental vehicle for job-related travel to patient locations, offices, and pharmacies.

Required Qualifications:
• Valid and unrestricted Registered Nurse (RN) license.
• BLS and/or CPR certification or willingness to obtain prior to employment.
• At least 1 year of RN experience in a clinical setting with infusion therapy exposure.
• Proficiency in electronic medical records documentation.
• Team-oriented mindset, excited to collaborate with healthcare professionals.
• Self-motivated and capable of working independently.
• Flexibility to accommodate varied work schedules.
• Reliable transportation for patient visits and other job-related travel.
Preferred Qualifications:

Your compensation will reflect your qualifications, local labor markets, and other relevant factors, with an hourly pay range of $28.94 to $51.63 based on full-time equivalency.
