Key responsibilities include: • Performing detailed OASIS assessments and ensuring accuracy in patient medication profiles • Promptly reporting lab values to the physician • Attending weekly case conferences and monthly management meetings • Consulting with physicians when necessary regarding patient care This part-time, salaried position requires a commitment of 24 hours per week over four days, and includes eligibility for our 60% paid days off program.
