As a Part-Time Registered Nurse, you will deliver and manage nursing care for patients in their homes as directed by a physician, adhering to all pertinent laws, regulations, and agency policies. You will play a key role in coordinating the complete patient care plan with other healthcare professionals and will facilitate communication to ensure continuity of care among physicians, agency staff, patients, families, and community resources.

Key Responsibilities:
• Deliver high-quality clinical services while adhering to infection control standards.
• Coordinate care with the full healthcare team from admission through discharge.
• Conduct clinical nursing assessments in line with federal/state program guidelines and payer requirements.
• Verify patient/client eligibility and medical necessity based on payer and agency policy.
• Create and modify personalized care/service plans in collaboration with community providers.
• Monitor the frequency and duration of care plans to meet patient needs and initiate updates with physician consent.

Required Qualifications:
• Active and unrestricted RN license in the state of practice.
• Current driver's license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
• Ability to provide care across a diverse patient demographic, irrespective of age, race, creed, color, sex, disability, or socioeconomic status.

Preferred Qualifications:
• Minimum of 1 year of RN experience.
• Capacity to work independently.
• Strong communication, writing, and organizational abilities.

Pay ranges from $28.27 to $50.48 per hour based on full-time employment.
