RESPONSIBILITIES: • Perform comprehensive assessments of residents' physical, emotional, and medical needs; monitor health status and communicate any changes effectively. • Administer medications and treatments as directed, ensuring adherence to physician orders and facility policies. • Monitor vital signs and conduct regular health checks; respond promptly to any changes in condition or emergencies. • Provide direct nursing care and assist residents with daily activities, including hygiene, mobility, and other personal care needs. • Collaborate with a multidisciplinary care team to ensure comprehensive, resident-centered care. • Maintain accurate documentation of all assessments, treatments, and communications regarding residents' medical records. • Effectively engage with residents, families, and the care team to discuss resident status, care plans, and any concerns. • Educate and support residents and their families regarding care plans, treatments, and wellness as needed. QUALIFICATIONS: • Valid, unrestricted RN license in Connecticut. • Graduate of an accredited nursing program (ADN or BSN); experience in long-term care or skilled nursing is preferred. • Current CPR/BLS certification as required by facility policy. • Strong clinical assessment skills, compassion, and a commitment to resident-centered care; excellent communication and interpersonal skills are essential. • Ability to thrive in a long-term care setting; reliable attendance and teamwork are crucial. • Capability to perform nursing procedures and respond effectively to changes or emergencies.
