Responsibilities: • Conduct assessments of residents’ physical, emotional, and medical needs; track health status and communicate any changes promptly. • Administer medications and treatments as permitted, following physician orders and facility policies. • Assess and record residents’ vital signs, perform health checks, and respond to changes in condition or emergencies proactively. • Deliver hands-on nursing care while assisting residents with daily living activities, hygiene, mobility, and more. • Work collaboratively with the care team—including CNAs, therapists, social workers, and physicians—to ensure holistic, resident-focused care. • Maintain accurate, timely documentation of assessments, treatments, and resident communications in medical records. • Effectively communicate with residents, families, and the care team regarding care plans, treatments, and any concerns. • Provide education to residents and families about care, treatments, medications, and wellness. Qualifications: • Valid, unrestricted RN license in Connecticut. • Graduate of an accredited nursing program (ADN or BSN); experience in long-term care is preferred. • Current CPR/BLS (or equivalent) certification as required by the facility. • Strong clinical assessment skills with a compassionate, resident-centered approach; excellent communication and interpersonal skills. • Proven ability to work in a long-term care or skilled nursing environment with reliability and teamwork. • Capability to perform nursing procedures, monitor residents’ conditions, and respond adeptly to changing circumstances.
