Your Responsibilities:
• Deliver skilled nursing care within patients' homes, including wound care, catheter management, medication management, diabetic care, and more.
• Conduct thorough assessments and develop personalized care plans that cater to the patient's needs.
• Educate patients and families with clarity and compassion—no confusing medical jargon!
• Document patient interactions efficiently using a user-friendly EMR system.
• Manage a variety of visits from routine wellness checks to complex case management.
