RESPONSIBILITIES:
• Conduct comprehensive assessments of residents' physical, emotional, and medical needs while monitoring their health status and reporting any changes.
• Administer medications and treatments as prescribed, ensuring adherence to facility policies and physician orders.
• Regularly monitor and document residents' vital signs and health status, responding promptly to any changes or emergency situations.
• Deliver direct nursing care and assist residents with activities of daily living, hygiene, mobility, and other care as needed.
• Collaborate closely with the care team—therapists, CNAs/LPNs, social workers, and physicians—to ensure cohesive, resident-centered care.
• Maintain accurate and timely documentation of assessments, treatments, and care provided in residents' medical records.
• Effectively communicate with residents, families/POAs, and care teams regarding resident status, care plans, treatments, and any changes or concerns.
• Educate and support residents and their families about care, treatments, medications, and overall wellness as appropriate.
QUALIFICATIONS:
• Valid, unrestricted RN license in Connecticut.
• Graduate of an accredited nursing program (ADN or BSN) with a preference for experience in long-term care or skilled nursing environments.
• Current CPR/BLS (or equivalent) certification as mandated by facility policy.
• Strong clinical assessment skills and a compassionate, resident-centered approach, along with excellent communication and interpersonal abilities.
• Capability to work in a long-term care or skilled nursing setting with a commitment to teamwork and reliable attendance.
• Proficient in performing nursing procedures including medication administration, IV therapy, and wound care while monitoring residents’ conditions effectively.
