
Patient Care Manager and Registered Nurse Hybrid in Chesapeake, OH
Job Description
In this unique Patient Care Manager RN Hybrid role, you will lead the supervision and coordination of clinical services, ensuring high-quality nursing care for patients in their homes as prescribed by their physicians. You will manage an interdisciplinary team, maintaining the continuity of care in line with the physician-prescribed plan and adhering to both state and federal regulations.
This hybrid position allows for flexibility with three to four in-office days per week, depending on location, offering an ideal work-life balance.
Key Responsibilities:
• Supervise and provide guidance to home health aides and LPNs, delegating tasks effectively.
• Deliver clinical services in accordance with the state nursing practice standards, the plan of care, and collaboration with healthcare professionals.
• Conduct comprehensive assessments (OASIS), medication reconciliations, and initial nursing evaluations.
• Manage patient referrals and clinician assignments, ensuring timely care initiation.
• Coordinate patient benefits, medical necessity, and insurance approvals.
• Continuously assess and personalize patient care, ensuring services align with physician orders and health eligibility criteria.
• Regularly review assessments and care plans, consulting with clinicians for recommendations as necessary.

Qualifications Needed:
• A current, unrestricted RN license in your state of practice.
• Valid CPR certification.
• A valid driver’s license, vehicle insurance, and reliable transportation.
• The ability to provide inclusive care regardless of a client’s background or circumstances.
Preferred Qualifications:
• Experience in home care settings.
• Ability to work independently and manage time efficiently.
• Strong communication, writing, and organizational skills.
Your salary will be competitive and based on various factors including local labor markets and experience. In addition to salary, we offer an extensive benefits package, including incentive programs, stock purchase options, and contributions to your 401k plan. The expected salary range is $58,800 to $105,000 annually for full-time employment.
