
Pediatric Home Care Registered Nurse in Delcambre, LA
Job Description
As a Pediatric Registered Nurse, you will provide and oversee nursing care for patients in their homes as directed by physicians, while ensuring compliance with applicable laws and agency policies. You will also coordinate care with other healthcare professionals involved in each patient’s treatment plan, facilitating continuity of care through close communication with physicians, agency staff, patients, families, and community resources.
Key Responsibilities:
• Deliver high-quality clinical services in alignment with your scope of practice and infection control standards.
• Collaborate with the patient’s care team from admission through discharge to coordinate care effectively.
• Conduct clinical nursing assessments according to federal and state regulations, as well as payer requirements.
• Verify patient eligibility and medical necessity for services as determined by payer and agency policies.
• Create and update individualized care/service plans in conjunction with other community providers.
• Ensure that the frequency and duration of care plans are tailored to meet patient needs, making revisions with physician oversight as necessary.

Required Qualifications:
• Current, unrestricted RN licensure in your state of practice.
• Valid CPR certification.
• Ability to function effectively in diverse home environments, accommodating all clients regardless of age, race, creed, color, sex, disability, or financial status.
• Current driver’s license, vehicle insurance, and reliable access to transportation.
Preferred Qualifications:
• 1+ years of RN experience, preferably in pediatric care.
• Demonstrated ability to work independently and effectively communicate, write, and organize.
State Specific Requirements:
LA:
• A minimum of one year of clinical experience as a Registered Nurse.
• One year of clinical experience may be waived if the Registered Nurse has recent clinical experience as an LPN.
• RN licensure should have no restrictions.
The hourly pay for this role ranges from $28.27 to $50.48 based on full-time employment, influenced by local labor markets, education, experience, and certifications.
