Position Summary
The Pediatric Palliative Care Nurse has the primary responsibility of providing care to patients and families on Navian Hawaii's Pediatric Hospice and Palliative Care Program.
Primary Responsibilities for Pediatrics
• Works with the physician and the Interdisciplinary Group to assess, plan, implement and evaluate the plan of care for patients and their caregivers.
• Functions as primary nurse within the framework of an Interdisciplinary Group providing essential patient care.
• Utilizes principles of palliative care and holistic philosophy to provide holistic care.
• The NCMs plans reflect skilled nursing judgment, medical necessity, measurable patient/caregiver objectives and reasonable goals.
• Provides case management services to best benefit patients and caregivers.
• Care provided reflects compliance with federal and state regulations, along with Navian Hawaiis policies and procedures.
• Documents all nursing and case management activities in accordance with hospice protocol and keeps attending physician informed of changes in patient condition.
• Requests support services from other Interdisciplinary Group members as appropriate.
• Demonstrates good judgment in seeking and accepting guidance; facilitates clear and honest communication with other team members.
• Ensures any follow?up needs are communicated to the after?hours team at the end of the day.
Knowledge and Skills Requirements
• Ability to exercise sound, independent judgment in planning and providing care; flexible and organized in planning.
• Ability to learn electronic medical record systems.
• Valid drivers license and use of a personal, insured, registered vehicle meeting Hawaii safety standards.
• Ability to set priorities, research, problem?solve and make independent decisions with minimal supervision by using logic to identify key facts, explore alternatives, and propose quality solutions within the scope of the job.
• Must be able to multi?task with the ability to meet multiple deadlines and possess strong organizational, planning and follow?through skills.
• Excellent written and verbal communication skills as well as the ability to communicate clearly, concisely, and effectively with all levels of individuals.
Experience and Educational Requirements
• A current Hawaii RN license in good standing. BSN preferred.
• At least two years of clinical experience in hospice and/or home health nursing preferred.
• At least two years of clinical pediatric experience preferred.
Working Conditions
• Indoors in an air?conditioned office as well as outdoors in all types of weather.
• Travel throughout Oahu and work on?site in numerous care settings including patient homes.
Employment Requirements
• Eligible for a legally compliant background check.
• Drug screen & physical exam.
• COVID vaccination.TB clearance.
• RN license.
• Providing copies of valid vehicle information such as auto insurance, inspection and registration.
• CPR certification.
Hours
• Generally, Monday?through?Friday, 8:00?am?through?4:30?pm.

Salary Range
• $95,000?to?$102,000?per year.
