
Per Diem Case Manager Registered Nurse in Columbia Heights, MN
Job Description
The Per Diem Case Manager Registered Nurse will play a crucial role in coordinating patient services through an interdisciplinary approach, focusing on clinical and psychosocial aspects of care. Engage in concurrent case management by assessing patients for admission appropriateness, continued hospitalization, and necessary care levels. You will facilitate timely care delivery by following CMS guidelines and collaborating with various stakeholders.
Location: This position may require support at any of the following locations:
• Waukesha Memorial Hospital - 725 American Ave, Waukesha, WI
• Oconomowoc Memorial Hospital - 791 Summit Ave, Oconomowoc, WI 53066
• Mukwonago Hospital - 240 Maple Ave, Mukwonago, WI 53149
Schedule: This is a Per Diem position, requiring at least one weekend shift per month.
Training: We provide two weeks of paid training during standard business hours (Monday to Friday).
Key Responsibilities:
• Lead patient disposition and utilization considerations with a focus on collaborative processes.
• Facilitate interdisciplinary teamwork and problem-solving for effective care delivery.
• Communicate effectively with staff, physicians, patients, and families.
• Model leadership through respect, courtesy, and efficiency.
• Coordinate care processes and identify opportunities for quality improvement.
• Educate patients and families, ensuring continuity of care and protecting patient rights.
• Engage in daily collaboration and assessment with patient care staff to optimize care levels.
• Manage schedules to enhance communication with physicians and other care teams.
• Maintain flexibility to support various departments as patient needs change.
• Develop discharge plans and facilitate the integration of post-hospital services.
• Ensure accurate documentation and communication of patient care plans.
• Adhere to relevant federal/state regulations and exhibit knowledge of managed care policies.

Required Qualifications:
• Associate's Degree in Nursing or higher
• Valid and unrestricted RN license in the state of employment
• At least 3 years of hospital and inpatient medical/surgical experience
• A minimum of 3 years working in inpatient hospital case management
• Experience with community resources and their policies
• Clinical experience across age groups from unborn children to geriatrics
Preferred Qualifications:
• Bachelor of Science in Nursing
• Case Management Certification
• Strong analytical and organizational skills
• Knowledge of Utilization Review and Medicare requirements
Compensation: The hourly pay range is $28.61 to $56.06 based on full-time employment considerations.
