Role Description This is a per diem Case Manager RN position with The Permanente Medical Group, Inc., based in Oakland, CA. This hybrid role allows for a combination of on-site and remote work. The Case Manager RN will be responsible for providing patient-centered care by coordinating discharge planning, utilization management, and case management services. The role requires collaboration with interdisciplinary teams to ensure patients receive timely and appropriate healthcare services, while supporting continuity of care and optimizing health outcomes. Qualifications • Experience in Discharge Planning and Utilization Management • Demonstrated clinical expertise in Nursing and Medicine • Proficiency in Case Management practices and principles • Ability to work independently and collaboratively in a hybrid environment • Professional Registered Nurse (RN) license in the state of California • Strong critical thinking, organizational, and communication skills • Familiarity with healthcare regulatory standards and compliance is a plus
