Perinatal & Pediatric RN | Cornerstone VNA Location: Community-Based – New Hampshire & Maine Service Area Job Type: Full-Time or Part-Time (Weekend & On-Call Rotation Required) Make a Life-Changing Impact for Families - From Newborns to Adults Cornerstone VNA is seeking a compassionate and skilled Perinatal/Pediatric RN to join our Home Care team. In this unique role, you will provide specialized care to perinatal and pediatric patients, while also caring for adult home care patients as part of a diverse clinical caseload. If you're passionate about supporting families-from newborn care and lactation support to caring for medically complex children and adult patients-this role offers meaningful, flexible, and independent nursing practice in the home setting. What You'll Do Perinatal & Pediatric Care • Provide direct nursing care to OB, perinatal, and pediatric patients. • Perform specialized skills such as heel sticks, phototherapy, lactation consultation, and newborn assessments. • Collaborate with interdisciplinary teams to support mothers, infants, and pediatric patients. • Support families through education, teaching safe care techniques, and reinforcing best practices. Adult Home Care Responsibilities • IMPORTANT: This position includes caring for both pediatric AND adult patients as part of the home care caseload. • Conduct skilled nursing assessments and develop patient-centered care plans. • Provide teaching, symptom management, and coordination of care for adult patients with varying conditions. Case Management & Care Coordination • Independently manage a diverse caseload across all ages. • Coordinate care with physicians, therapists, social workers, and other disciplines. • Ensure accurate and timely documentation in the EMR. • Arrange referrals and participate in interdisciplinary care planning. Clinical & Regulatory Requirements • Complete OASIS assessments accurately (RN only). • Follow home care regulations, best practice guidelines, and agency procedures. • Educate patients and caregivers regarding safe home care, treatments, medications, and disease management. What You Bring • Current NH or Compact RN license. • Pediatric experience required/preferred: 2+ years strongly preferred. • Home care experience: 1+ year preferred. • Strong assessment, teaching, and independent decision-making skills. • Comfort working with newborns, children, and adults. • Ability to manage a varied and flexible home care schedule. • Compassion, patience, and excellent communication skills.
