• Support Home Therapy: As a Home Therapy nurse, you will train patients in peritoneal dialysis or home hemodialysis both in-center and in patients’ homes.

The Home Therapy Registered Nurse, in compliance with federal and state regulations, provides patient training and support for those choosing a home dialysis modality. You will be responsible for ensuring quality care and superior customer service for all patients.
Key Responsibilities:
• Adhere to all federal, state, and local laws and regulations applicable to home dialysis care.
• Train patients and/or care partners on self-care practices for peritoneal dialysis or hemodialysis.
• Conduct home visits to assess the patient’s environment and evaluate their progress regularly.
• Effectively communicate assessment results and patient needs to the healthcare team.
• Document all interactions and patient care in the Electronic Medical Record to ensure continuity of care.
• Participate in infection control measures and stay prepared for emergency protocols.
• Ensure clinic and patient supply inventory meets patient needs efficiently.
• Continuously improve patient outcomes through analysis and quality improvement efforts.
• Carry out and document laboratory tests as ordered by physicians and alert them of critical values promptly.
• Review and document patient medication profiles, administering medications as directed.
• Participate in on-call services as needed and exhibit flexibility to meet patient and program demands.
• Maintain reliable attendance for the position.
For Home Hemodialysis:
• Coordinate with technical services to ensure proper equipment operation in patients’ homes.
• Conduct required water sampling per policy and monitor results.

Qualifications:
• Current RN license in good standing is required.
• Minimum of 12 months nursing experience, with 3 months in the specific dialysis modality.
• Experience with maintenance dialysis patients is preferred.
• Current CPR certification is required.
• Ability to distinguish all primary colors is necessary.
• Successful completion of orientation and ongoing competency is expected.
• Strong communication skills in English, both verbal and written.
• Basic computer skills, with proficiency in clinical applications required within the first 90 days.
• Meet any applicable state practice requirements.
