This is a benefit eligible part time position (24 hours per week - 3 days) working on-site at our Heart of the City Health Center - 100 Cherry St Se.
Overview Of Position
Under the direction of the Chief Medical Officer (CMO) or applicable Medical Director and with guidance from a collaborating physician, the Nurse Practitioner / Physician Assistant is responsible for medical care consistent with accepted professional standards.
Must adhere to the organizational Policies, Procedures, Code of Ethics, and all licensing and accreditation standards of the position. Must represent the organization’s Mission in all actions and communications.
Essential Duties And Responsibilities
• Provide excellent direct patient care in a comprehensive, compassionate, and team-based manner.
• Assist in the coordination and/or supervision of prevention services for patients.
• Provide medical evaluation and treatment of ill patients based on scientific knowledge.
• Provide medical care to all patients within the expectations of the specialty hired into.
• Family Practice: Primary care for all ages with a focus on prevention and disease management.
• Adult Medicine: Primary care for adults (17+) with a focus on prevention and disease management.
• Pediatrics: Primary care for infants, children, and adolescents with a focus on prevention and disease management.
• OB/GYN: Obstetric and gynecologic care for all with needs.
• Maintain accurate and detailed medical records for each patient.
• Prescribe medication, referrals, and diagnostic studies when indicated.
• Recommend lifestyle changes as appropriate to improve health and quality of life.
• Follow and interpret diagnostic results, ensuring that patients are notified of results and next steps.
• Promote team-based care to better serve patients, including collaboration with MA, nursing staff, RD, pharmacist, community health workers, behavioral health, optometrists, and dentists.
• Provide coverage for patients when other providers take leave.
• Participate in after-hours call.
• Participate in regular chart review with collaborative physician per agreement.
• Participate in the peer review process.
• Follow clinic policies, procedures, and protocols to achieve Cherry Street Services Inc. goals and objectives.
• Strive to achieve high-quality metrics under Cherry Health’s Quality Assurance program.
• Serve as a liaison with the medical community and maintain linkages with external agencies and organizations.
• Attend regular internal and external meetings to support clinical program and agency goals.
• Understand and adhere to CSHS’s compliance standards, Corporate Compliance Policy, Code of Conduct, Conflict of Interest Policy, and HIPAA.
• Keep abreast of all pertinent federal, state, and CSHS regulations, laws, and policies as they evolve.
• Perform other duties as assigned.

Two years’ experience working in a primary care clinic setting as an advanced practice provider required or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.
Physical Requirements
Possible exposure to patients/clients with illnesses and/or infections. Frequent telephone and computer use, requiring manual dexterity for writing and computer use. Sitting, standing or walking for extended periods of time. Specific vision abilities required by this job may include close vision, distance vision, and ability to adjust focus. May require bending, squatting, or kneeling and lifting up to 50 pounds. Use of Personal Protective Equipment (PPE) as required.
