Our platform automates charting and compliance so you can focus on high-quality senior care.Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton. Immediate need: ~300 eligible residents are ready to be seen across our Stockton assisted living communities, providing steady, predictable volume.What You’ll DoRoutine & urgent assessments; chronic disease managementPost-discharge follow-ups, med reconciliation, labs/diagnosticsCoordinate with facility staff, specialists, families/POAsSame-day documentation; CMS/regulatory complianceSchedule & Setting Part-time; weekday blocks preferred. Clustered Stockton routes to minimize travel.
