This role offers steady, predictable patient volume and the opportunity to deliver high-quality care directly where seniors live. What You’ll DoRoutine & urgent assessments; chronic disease managementPost-discharge follow-ups, med reconciliation, labs/diagnosticsCoordinate with facility staff, specialists, families/POAsSame-day documentation; CMS/regulatory complianceSchedule & Setting Full-time; weekday schedule preferred. Routes are clustered by region to minimize travel.
