
Private Duty Registered Nurse in Eldorado, WI
Job Description
As a Private Duty Registered Nurse, you will be essential in providing high-quality, personalized nursing care to patients in their homes as prescribed by their physician and in accordance with all applicable regulations and agency policies. You will collaborate with other healthcare professionals to enhance patient care continuity by planning and sharing information effectively.
Key Responsibilities:
• Deliver exceptional clinical services while adhering to infection control standards.
• Coordinate comprehensive care with the patient’s healthcare team from admission through to discharge.
• Conduct clinical nursing assessments as required by federal and state programs and payers.
• Ensure that patients meet eligibility and medical necessity requirements for services according to agency policy and payer guidelines.
• Develop and adjust individualized care plans in partnership with community health providers.
• Monitor and ensure the frequency and duration of care plans meet the patient's needs, making revisions with the physician’s consent.

Requirements:
• Valid and unrestricted Registered Nurse license in your practice state.
• Current driver's license, vehicle insurance, and reliable transportation.
• Current CPR certification.
• A commitment to providing care in any home setting, regardless of client background or circumstances.
Preferred Qualifications:
• At least 1 year of RN experience.
• Able to work independently and effectively.
• Strong communication, writing, and organizational skills.
Compensation for this role ranges from $28.27 to $50.48 per hour, depending on experience, education, and local market conditions. Our comprehensive benefits package includes incentive and recognition programs, equity stock purchase options, and a 401k plan, subject to eligibility requirements.
