Job Description
As a Private Duty Registered Nurse, you will provide and direct nursing care to patients in their homes, as prescribed by physicians and compliant with regulations and agency policies. Your role will involve coordinating comprehensive care plans alongside healthcare professionals to ensure seamless patient care, through communication with physicians, agency staff, patients, families, and community resources.
Primary Responsibilities:
• Deliver top-quality clinical services in accordance with infection control standards and scope of practice.
• Collaborate and coordinate care with the health team from admission through discharge.
• Conduct clinical nursing assessments based on federal/state requirements and payer guidelines.
• Confirm patient/client eligibility and medical necessity for required services per payer and agency policy.
• Create and update customized care/service plans with community healthcare providers.
• Adjust the plan of care as necessary, ensuring it aligns with patient needs, with the physician's approval.

Required Qualifications:
• Valid and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance, and reliable access to a vehicle or public transport.
• Current CPR certification.
• Ability to adapt to diverse home environments regardless of age, race, creed, gender, disability, or financial status of the client.
Preferred Qualifications:
• 1+ years of RN experience.
• Capability to work independently.
• Strong communication, writing, and organizational skills.
Compensation for this position is based on various factors such as local labor markets, education, experience, and certifications. We offer an extensive benefits package, incentive and recognition programs, stock purchase options, and 401(k) contributions (subject to eligibility). The hourly pay for this role ranges from $28.27 to $50.48 per hour for full-time employment. We adhere to all applicable minimum wage laws.
