Every visit you make helps patients feel safe, respected, and empowered in the place they call home. ------------------------------------------------------------------------------------------------------------------- Why Join Elara Caring? • Flexible PRN scheduling and work autonomy • 1:1 patient care with time to truly make an impact • Supportive, collaborative clinical leadership • Competitive compensation ------------------------------------------------------------------------------------------------------------------- What You’ll Do • Provide skilled nursing care and oversight in the home setting • Conduct start-of-care visits and ongoing assessments in compliance with State and Federal regulations • Develop, review, and update individualized plans of care • Supervise and support home health aides and personal care staff at required intervals • Monitor and report changes in patients’ physical or mental status • Educate patients, caregivers, and staff using sound teaching and clinical judgment • Ensure patient safety by assessing home environments and addressing potential hazards • Maintain accurate documentation and ensure compliance with HIPAA and regulatory requirements • Participate in staff meetings, in-services, and continuing education • Promote Elara Caring’s mission, values, and commitment to quality care ------------------------------------------------------------------------------------------------------------------- What You’ll Need • Graduate of an accredited Certificate, Diploma, Associate, or Bachelor’s School of Nursing • Active State RN license • Minimum of 1 year of clinical nursing experience • Home health or hospice experience preferred • Strong clinical assessment and decision-making skills • A genuine passion for patient-centered care • Reliable transportation and ability to travel (approximately 50%) ------------------------------------------------------------------------------------------------------------------- At Elara Caring, you’re not just filling shifts, you’re fulfilling a calling.
