Position Overview:
As a PRN Home Infusion Nurse, you will be a vital part of our patient care team, delivering nursing care to patients in their homes or alternate settings. Working closely under the supervision of a designated supervisor, you'll ensure patient safety and compliance with physician care plans while providing compassionate support and education to patients and their families.
Key Responsibilities:
• Develop and implement patient care plans while actively evaluating patient progress and modifying treatments as necessary.
• Conduct thorough assessments of patient needs and physical status, ensuring effective communication with physicians about any significant changes.
• Educate patients and their families to promote independence in managing prescribed therapies.
• Document patient care accurately and timely to ensure continuity of care and compliance with billing practices.
• Assist in discharge planning, preparing comprehensive summaries and patient instructions to ensure smooth transitions in care.
• Maintain adherence to Naven Health policies, professional standards, and regulatory guidelines at all times.
• Utilize technology effectively in managing patient care and documenting services provided.
• Respond promptly and appropriately to patient needs and perform emergency procedures when necessary.
• Uphold ethical standards and ensure patient confidentiality in accordance with HIPAA regulations.
• Communicate knowledgeably about Naven Health's services to educate patients about their care plans.
• Participate in training, mentorship, and orientation for new nursing staff as required.
• Engage in required on-call responsibilities as needed.
• Take on additional duties as assigned by your supervisor.
Requirements:
• Active RN license without restrictions in your practicing state.
• 0-2 years of nursing experience, preferably in infusion nursing.
• Current CPR certification.
Preferred Qualifications:
• Bachelor of Science in Nursing from an accredited institution.
• At least 1 year of experience as a registered nurse in home health or acute care settings.
