Type of Opportunity: PRN Work Shift: Varied Days and Hours Responsibilities: • Act as a clinical resource, overseeing patient care delivery, directing and delegating assignments to home health aides based on patient needs. • Maintain clinical competency in direct patient care, evaluating outcomes, consulting specialists as needed, and adjusting care processes for optimal outcomes. • Collaborate effectively with patients, families, and multidisciplinary teams to coordinate comprehensive care. • Provide education and support to patients and families regarding their rights, responsibilities, and care processes. • Assess educational needs and deliver hands-on teaching using various aids in the home setting. • Stay updated with current nursing research, practice standards, and hospital policies for improved patient care quality. • Communicate effectively within the nursing team and tackle problem-solving and conflict resolution to enhance team dynamics. • Perform on-call duties, triaging patients over the phone and visiting as necessary. • Supervise home health aides per Medicare/Medicaid regulations. • Undertake additional functions as required to support patient care. Qualifications: Licensure: State of New Mexico or Compact State Nursing License or Graduate Nurse Permit required. Academic Preparation: Degree from an accredited nursing program (if a program is under conditional status, approval may be granted by system CNO). Minimum ADN with an expectation to complete a BSN within five years of hire.
