Job Summary and Qualifications We seek a Registered Nurse (RN) Case Manager to deliver clinical expertise in ensuring patients receive high-quality care efficiently. Your skills and knowledge will be crucial in positively impacting patient outcomes. Your responsibilities will include: • Coordinating patient care plans, managing lengths of stay, and developing safe discharge plans. • Facilitating care transitions and identifying patients at risk for adverse outcomes. • Conducting comprehensive assessments of patients' psychosocial and medical needs. • Collaborating with Social Services to address post-acute care needs. • Utilizing evidence-based tools and criteria to evaluate care progression. • Making appropriate referrals for ongoing case management and chronic disease support. • Acting as an advocate and effective communicator among patients, families, medical staff, and external agencies. What you will need: • A degree from an accredited Nursing program; a BSN is preferred. • Active RN license in your practicing state or a Compact/Multi-State RN license. • 1-3 years of clinical nursing experience required; 2 years in Case Management preferred. • Certification in Case Management or Utilization Review is a plus. • Experience with InterQual criteria is preferred.
