Job Description
As a Registered Nurse, you will be instrumental in delivering and overseeing nursing care to patients in their homes, following physicians' prescriptions and adhering to relevant laws, regulations, and agency policies. You will coordinate the overall care plan with healthcare professionals to ensure seamless patient care, which involves communication with physicians, agency staff, patients, families, and local resources.
Primary Responsibilities:
• Deliver high-quality clinical services adhering to infection control standards and practicing within your scope.
• Coordinate comprehensive care with the patient's healthcare team from admission to discharge.
• Perform clinical nursing assessments in accordance with federal and state regulations.
• Verify patient eligibility and medical necessity for services in line with payer and agency policies.
• Develop and adjust individualized care plans in collaboration with community providers.
• Ensure that the frequency and duration of care meet patient needs and make revisions with physician approval.

Required Qualifications:
• Current and unrestricted RN licensure in your state.
• Valid driver's license, vehicle insurance, and a reliable vehicle or access to public transport.
• Current CPR certification.
• Ability to work in diverse home situations regardless of the client's background.
Preferred Qualifications:
• At least 1 year of RN experience.
• Ability to work independently.
• Strong communication, writing, and organizational skills.
The pay rate for this role is competitive, ranging from $28.27 to $50.48 per hour, adjusted based on various factors such as local market rates, experience, education, and certifications.
