Schedule: Flexible, 35 hours/week
Compensation: $90,000–$95,000 per year + comprehensive benefits

About the Role
We are seeking an experienced and compassionate Registered Nurse (RN) to support a Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). This role provides essential nursing services to individuals with intellectual/developmental disabilities (IDD) and co-occurring behavioral health needs.
The RN will play a key role in crisis stabilization, transitional planning, skilled nursing care, and care coordination as individuals move from inpatient settings back into the community.
Key Responsibilities
Clinical Care
• Conduct admission screenings and serve as an active member of the intake team.
• Perform initial and ongoing nursing assessments.
• Oversee medication administration, monitor therapeutic responses, and ensure compliance with regulations.
• Provide direct nursing care including wound care, diabetes management, respiratory support, and chronic condition oversight.
• Identify early signs of medical or psychiatric crises and initiate interventions.
• Maintain and review vital health logs and documentation.
• Manage medication storage, disposal, and ordering procedures.
• Develop and monitor personalized health goals.
• Ensure compliance with infection control and universal precaution guidelines.
• Maintain first aid supplies and medical inventory.
• Provide 24/7 phone availability for urgent consultation.
Case Management & Transitional Support
• Collaborate with ICM teams to address medical and behavioral health needs.
• Provide health education to individuals, families, and support staff.
• Support hospital discharge planning and community reintegration through CTI.
• Coordinate care with primary providers, specialists, and community resources.
Collaboration & Training
• Serve as a clinical liaison among staff, families, and external providers.
• Participate in case conferences and interdisciplinary planning.
• Train and mentor direct support professionals (DSPs) on health tasks and emergency response.
• Complete timely and compliant documentation under OPWDD, OMH, and DOH regulations.
Qualifications
• Active, unrestricted RN license in New York State.
• Minimum 2 years of nursing experience (IDD, behavioral health, crisis, or community health experience preferred).
• Strong clinical assessment, crisis intervention, and care coordination skills.
• Excellent communication, organizational, and documentation abilities.
• Ability to work independently and collaboratively with multidisciplinary teams.
• Valid driver’s license preferred (travel between sites may be required).
