Finally, being patient-centered, we prioritize the needs and well-being of those we serve, striving to exceed expectations and make a meaningful difference in their lives.    A Day in the Life of this Role:    Reporting to the Associate Director of Psychiatry, the Psychiatric Registered Nurse serves as the Psychiatric providers’ clinical resource responsible for professional communications regarding patient care, response to clinical messages such as patient calls or web communications, and telephone triage. This position also works to ensure that patients are able to access services in accordance with quality guidelines.  Summary of Duties:  Manages calls to Psychiatric providers which require clinical knowledge; may address issues directly, generate a Telephone Encounter in EPIC, or contact the psychiatric providers directly.  As a member of the psychiatry team, functions in the role of patient advocate by being in the position of initial contact for patient-related issues.  Conducts telephone triage for patients of all OCHS psychiatric providers. Documents triage calls thoroughly and concisely, including triage assessment, plan, implementation, evaluation, and follow-up.  Manages appropriate medication refill requests; evaluates status of request, last office visit, next office visit, and any labs appropriate to the medication. Sets up prescription for providers review to send forward to pharmacy.  Assures that pertinent medical information is forwarded to proper individuals for continuity of patient care.  Performs excellent patient care and listens to patients’ concerns and communicates pertinent information to clinical staff.  Review and triage incoming referrals to the psychiatry department. Determine their appropriateness and assign them to the appropriate provider  Conduct nursing visits to discuss patient concerns, provide psychoeducation, administer medication injections, sign controlled substance contract or other tasks that would require a nursing visit to be conducted.  Discuss clinical concerns with the appropriate provider or the associate director of psychiatry  Attend regularly scheduled psychiatry meetings  Manages time effectively and efficiently in order to meet the demands as a clinical resource.  Performs other related duties as required or assigned  May occasionally function as a primary care team RN in the Health Center setting in a backup capacity.  Other related duties and projects as assigned.   QualificationsWhat we need from you:  Successful completion of an accredited school of nursing and a current active RN license from the Massachusetts State Board of Registration required.  BLS Certification required.  Minimum of three years’ professional nursing experience required, preferably a psychiatric setting.  Minimum of one year of telephone triage experience required, preferably in medical practice setting.  Knowledge of mental health diagnostic criteria, psychotropic medications, community health center/ambulatory care environment, telephone-based clinical assessment techniques, and basic knowledge of ACO relationships and expectations are all required.  Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.  Excellent written and verbal communication skills – bi-lingual helpful  Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.  Excellent judgment and ability to solve problems in a timely manner.  Ability to adhere to strict confidentiality standards.  Demonstrated ability to work effectively both as a team member and independently.  Demonstrated ability to work in a culturally diverse and inclusive environment.    AAP/EEOC Statement   Outer Cape Health Services is committed to providing equal employment opportunity in all our employment programs and decisions.
